Ihren Durchbruch hatte sie 1980 mit „Double Negative“ und der Horrorkomödie „Beetlejuice“. Besondere Aufmerksamkeit erlangte sie durch ihre Darstellung der vergesslichen Mutter von Macaulay Culkin in den Filmen „Kevin – Allein zu Hause“ (1990) und „Kevin – Allein in New York“ (1992). O'Hara gewann unter anderem den US-amerikanischen Emmy-Fernsehpreis für ihre Rolle in der TV-Comedy-Serie "Second City Television". Ihren zweiten Emmy Award erhielt sie 2020 als Hauptdarstellerin in der kanadischen Sitcom "Schitt’s Creek".
